This document reflects information whether seen or displayed from the hospital or IDPH EMS staff perspective. It does not indicate exact implementation of features or data storage. Implementation will be coordinated with IDPH Information Technology staff.
Several fields have been identified as repeat values. These fields need to be duplicated as many times as the type of field indicates. The fields identified in the templates are inter-related and should be duplicated. Refer to the Appendix 5.b. (Screen Layouts).
The Data Element descriptions include the following information:
Registry Section - The screen where the element will be entered or displayed.
Field Name - The descriptive name of the element.
Data Field Name - The suggested database name for the element. The field name and table implementation may be different than depicted on the following pages, based on IDPH standards. The suggested data field name will be used to simplify user understanding, especially in the report environment. Mapping may be required to link the suggested database name for the element in this document to the actual data field names in the database.
Accessible by IDPH - Indicates if the element will be viewed/accessed by IDPH EMS staff. Some elements will only be used by the hospital. If the field is not accessible by IDPH, it should be stored in encrypted format.
Max. Length of Field - The suggested length of the element.
Type of Field - The suggested data type of the element. Types include Char (character), Numeric, Decimal, VarChar (variable length), Code (V) (code table validation field), Date, and Time. Internal format of all fields will be based on IDPH standards. Data entry of date and time fields will be in MM/DD/YYYY HH:MM (military time) format.
Value Range - The value range of the data element. Blank or None indicates there is no value range for the field. PICK ALL THAT APPLY is an option for some CODE (V) fields. Code, numeric and date/time fields have been given the values of UNK and NA. Although code, numeric, and date/time fields cannot contain these values, implementation must include a way to indicate that the hospital has entered UNK (Unknown) or NA (Not Applicable) for these fields. These fields will be identified by the symbol (^). Numerically calculated fields may have an (*) to indicate insufficient data information entered for the fields involved or needed for calculation.
Definition - The definition of the data element. This will be the basis of the User Manual definitions.
Value - The values and descriptions for data elements or a reference to the appendix for larger table references.
